Therapeutic Day Rehabilitation Program (TDR)
Monday, Tuesday and Thursday
99 Nixon Street, Shepparton VIC 3630Is your alcohol or other drug use causing problems for you or those you love?
We understand the difficulties you face and want to come along-side you to help you build your skills and resources to live the life you want.
TDR is a 10-week intensive non-residential rehabilitation group program is available for anyone wishing to address their substance use. The program is based on a recovery model of care and seeks to develop life skills including communication, coping strategies and relapse prevention.
All sessions operate in school hours to allow parents and care givers to take part. The program is free and provides a daily lunch and a recreation component.
Delivered in partnership with GV Health and Odyssey House.

Housing Support for the Aged Program

27 Wyndham Street, Shepparton VIC 3630The Housing Support for the Aged Program (HSAP) supports clients aged 50 years and over who have complex health needs and a history of homelessness and are entering public or community managed housing, (or) already in public housing if their tenancy is at risk. It helps clients maintain long-term public housing and improve their health and wellbeing. HSAP sits within CCP as an internal target.

Homelessness Support Services Program

27 Wyndham Street, Shepparton VIC 3630The Homelessness Support Services Program (referred to by DFFH as “Homelessness Transition Support”) provides assessment, referral and case managed support which may include guidance, crisis resolution, personal care, life skills training, information and advocacy and/or assistance with accessing appropriate long term housing and training and employment opportunities.
Homelessness transition support can be provided, irrespective of a person’s existing living arrangement for example in rooming houses, transitional housing, squats and informal arrangements.
Support aims to achieve long-term, sustainable housing, employment, education, health and wellbeing outcomes for people who are homeless or at risk of homelessness.

Health Access Program
Thursdays, 12.00pm - 2.00pm
99 Nixon Street, Shepparton VIC 3630The Heath Access Program has been created to provide medical care and support for those who may be experiencing or at risk of homelessness.
Experienced doctors will provide primary care and facilitate access to other health services for those with specific or complex heath needs.
There is no cost associated with this program.
In partnership with Wyndham House Clinic.

Community Connections Program
Monday to Friday, 9.00am - 5.00pm
27 Wyndham Street, Shepparton VIC 3630The Community Connections Program (CCP) helps Victorians with multiple or complex needs who are homeless or live in low cost accommodation. Often these people are isolated and not well connected with health, housing or community services. They easily 'slip through the gaps'. CCP providers proactively seek, find, engage, assess and link these people with the services they need. Each provider has a pool of funds to help people overcome a crisis or a pressing need. Service providers can also help people relocate if their accommodation closes.

Care Finders
Monday to Friday, 9.00am - 5.00pm
27 Wyndham Street, Shepparton VIC 3630Care Finders (CF) support vulnerable older Australians (65 years and older) who have one or more reasons for requiring intensive support to interact with My Aged Care and access aged care services and connect with other relevant supports in their community. To receive care finder support, a person must have no carer or support person who can help them, or not have a carer or support person they feel comfortable or trust to support them, and be eligible for government-funded aged care.

North East Youth Services - Youth Participation & Life Skills
Monday to Friday, 9.00am - 5.00pm
Greater SheppartonYouth Participation team members work alongside young people, so that we can see young people participating in change within the service and in greater Shepparton. This includes a YAG and young people working towards advocacy events for what they see is important (like Youth Homelessness Day). We are also engaged with schools and support the curriculum in delivering information about topics that the curriculum doesn't necessarily include (e.g. cybersafety). Youth Participation aims to engage and empower young people to create youth input and positive change within their community.
Our Life and Living Skills worker also facilitates well being programs such as Positive Lifestyle Program, Rage! and others to engage the young people that live on site. There is also school holiday programs that take young people to do things like go to the Cinema, day trips and fun-filled activities. -
North East Youth Services - Youth Justice

Wallan to WodongaYouth Justice Group Conferencing and Diversion
If you are required to present at Children’s Court you may be given the opportunity to participate in a Youth Justice Group Conference or Youth Diversion Conference. Participation in either of these programs is voluntary.
The Conference is your chance to talk about what happened, consider the impact of the offending, and how to repair the harm caused. You will be asked to participate in some preparation sessions with the convenor, and asked to identify people who are important to you who can support you at the conference. The Salvation Army also run restorative conferences for youth in the community. If you have a relationship you would like to repair, please contact our convenors to discuss eligibility and possibilities.
Referrals for Group Conferencing and Diversion occur via the
Court system.Youth Justice Community Support Service
Youth Justice Community Support Service (YJCSS) is an integrated approach to the provision of intensive support and services to Youth Justice clients to complement the statutory case management undertaken by Youth Justice units. The service has been developed recognising that Youth Justice clients present with a range of complex and varied needs that require an
individualised service response.Referrals into the Youth Justice Community Support Service
occur via the Department of Justice and Regulation Victoria. -
North East Youth Services - Youth Homelessness and Housing Support
Monday to Friday, 9.00am - 5.00pm
Greater SheppartonCase Managers provide support to young people aged 16-25 who are homeless or at risk of homelessness. This can include situations such as couch surfing, living in unsafe, unsustainable or temporary accommodation or experiencing family violence.
Young people are supported to actively work toward achieving their ideal housing goals. The program further supports young people in addressing issues that may have contributed to their situation, including family reconciliation, referral to specialist services and support to access external supports.
Referral is via Beyond Housing, on 03 5833 1000.

North East Youth Services - Accommodation
Monday to Sunday, 24 hour
SheppartonThe Salvation Army Northeast Youth Services provides youth accommodation for young people aged 16-25.
Located within Shepparton, the Youth Accommodation Program allows a young person to stabilise their situation and receive support to set goals for the immediate future. Qualified Youth Workers are on-site 24 hours and support young people within a therapeutic framework.
The Youth Accommodation Program is funded to provide up to eight weeks of accommodation, during which time young people are supported to work toward finding sustainable housing.
Where the young person doesn’t have professional supports or a Case Manager in place, they will be appointed a Case Manager to support them in achieving their housing and goals.
While residing in the accommodation young people are expected to engage with the community, including connecting with professional services, continuing education, job seeking etc.
