Community Connections Programme

An assertive outreach support program assisting and responding to the support and housing needs of people at risk of homelessness who are residing in low cost housing, including boarding houses.

Target Group:

Consumers who are homeless or at risk of homelessness who have a disability and may have a range of complex and/or unmet needs including:

  • Frail aged
  • Drug and alcohol
  • Mental Illness
  • Intellectual Disability
  • Acquired Brain Injury
  • Dual Disabilities
