The Continuing Care program is a new initiative by Westcare’s Transitional Services. Continuing Care aims to provide additional support to young people transitioning to independent living. In addition to this, Continuing Care offers case management or support to young people (up to 25 years old) that have exited Westcare's services yet require continued care.
The program will work alongside all Westcare services to provide them with appropriate Resources to support young people as well as ensuring that young people have appropriate continuing care and exit plans.
Continuing Care has developed a Life Skills Program and a mentoring program will follow shortly, to provide young people with the skills and support they require to live independently.
It will seek to build partnerships with current Leaving Care services and provide referral support to case management teams and young people.
The Continuing Care Case Manager will address the importance of preparing young people for independence, assist them to develop stronger community linkages and provide young people who have left care with case management support.
