Community Connections Program

Community Connections Program (CCP)

An assertive outreach support program assisting and responding to the support and housing needs of people at risk of homelessness who are residing in low cost housing, including boarding houses.  This program includes “Living Well” which provides a range of home care services to marginalised and disadvantaged people.

Target Group:

Consumers who are homeless or at risk of homelessness who have a disability and may have a range of complex and/or unmet needs including:

  • Frail aged
  • Drug and alcohol
  • Mental Illness
  • Intellectual Disability
  • Acquired Brain Injury
  • Dual Disabilities

  • Advocacy
  • RDNS (Nurse)
  • Case Management 

 

Living Well Program

The Living Well Program aims to provide clients (who are HACC eligible) with the necessary expertise to remain in their home.  This program targets consumers who have complex needs whose environment, living circumstances and/or behaviours create a barrier for HACC service provision. After interventions the goal is to refer back to main stream HACC services.
