If you would like a response please provide your contact details using the online feedback form below. Upon receipt of your complaint, you will contacted within two business days by someone who can work with you to address your concern or dissatisfaction.


Alternatively, if you wish to submit feedback anonymously, please return a completed copy of this form not including your signature, name or contact details. This form can be returned directly to Westcare or any other Salvation Army location. Please be aware that by issuing an anonymous complaint, it will limit our ability to conduct a formal investigation and resolve the matter to your satisfaction (where possible).
