Intensive Case Management Initiative

What we aim to do: To assist clients with high and complex needs, to access, establish and maintain appropriate accommodation and links with services through intensive case management and service co-ordination.

Who is eligible? Those homeless or at risk of homelessness, 18+ years old with high and/or complex needs

Location of services: West Hume Area, including Greater City of Shepparton and the Shires of Moria, Mitchell, Murrindini and Strathbogie.

More about this service: Pathways Intensive Case Management aims to provide the right mix of housing and support where and when it is needed by individuals and families who are homeless or at risk of becoming homeless. This means tailoring our response by providing services and intensive support programs to best meet someone’s needs.  We assist in emergency accommodation, material aid and personal support to those who literally have nowhere else to go. The support we provide includes counselling, material aid, financial aid, assistance with education and training as well as referrals to specialist health services.

Pathways Intensive Case management aims is to work with people at the earliest stage possible to prevent them from becoming homeless. Pathways place a high value on early intervention and preventative work to assist people before they need crisis support. We aim to provide more than just shelter. Encouraging the participation of individuals and families in their local communities, schools, education, training and paid work is what we are about. Our way is to assist people to overcome the issues that led to their housing crisis or homelessness and to ensure they participate and engage in community activities. This assists in removing barriers of seclusion and isolation that often arise during or after their homelessness or housing crisis.
