Rural City of Mildura, Rural City of Swan Hill, Shire of Buloke, Shire of Gannawarra, Shire of Loddon, Shire of Campaspe, Greater City of Bendigo, Shire of Central Goldfields, Shire of Mount Alexander and Shire of Macedon Ranges. 

Primary office locations (Delivery Monday—Friday):

Secondary locations (Delivery 1-2 days per week):

Rochester, Echuca, Kyabram, Maryborough, Castlemaine, Gisborne, Robinvale, and anywhere else across the Loddon Mallee close to where you live.

About the three streams

Counselling stream

This program provides a range of face-to-face, telephone, individual and group counselling interventions for clients and their families. 

  • A range of evidence-based counselling approaches tailored to client and family needs and preference.
  • Recovery-oriented counselling practice underpinned by a strengths-based approach to building resilience for clients and their families.
  • Counselling interventions targeted to extend clients’ and family members progress towards achieving recovery goals negotiated at assessment, and re-assessed at points of treatment review.
  • Exit/post-treatment support plans are integrated as part of the counselling process, including counselling provided as part of a longer-term Care and Recovery Plan.
  • Liaison with specialist services, such as Aboriginal Health and ABI to build capacity for practice that is responsive to the needs of these client groups, and provides the availability of secondary consultation throughout the catchment area.
  • Client follow-up at 3 and 12 months post counselling, (or more frequently if required, as part of post-treatment planning) to ensure that their recovery and discharge plans remain relevant and effective. Follow-up provides an opportunity for re-engagement if needed. 
  • Clients may be self-referred, referred by significant others, by other agencies or may be mandated to engage in treatment as part of community corrections order, parole or bail conditions, other diversion programs and or the Step Out Program. Back to top

The Care & Recovery Coordination stream

This program provides a range of face-to-face, telephone, individual and interventions for clients and their families:

  • Coordinates Care for people looking for support around issues caused by Substance use
  • The AOD Practitioner works with clients to develop a Recovery Plan which identifies treatment pathways and clearly defines goals. This is achieved through utilising a person-centred approach; clients will be supported to take an active and equal role in their treatment planning, with treatment customised to their multiple needs with consideration given to family, significant others, cultural circumstances and not just their substance use.
  • The AOD Practitioner assists the client to access to the range of health and social care services, including homeless-specific services if required; providing brief interventions such as group work and relapse prevention programs and linkages to peer support programs.
  • The AOD Practitioner will maintain regular contact with clients and families, by telephone, in person and through outreach visits, and will deliver a range of evidence-based interventions that will assist in maintaining clients’ motivation to change and support clients through their recovery journey.
  • The CRC stream also includes the nomination rights to 7 Transitional properties located in Bendigo that Haven Home safe property manage. SalvoCare Northern AoD services are responsible for providing Alcohol and other Drugs treatment through the Care & recovery coordination stream in addition to developing long-term housing plans for the tenants.

The Assessment stream

This program provides face to face and telephone comprehensive assessments for people wanting to address their substance use. The assessment Stream:

  • Provides an entry point to all of the AOD services available for people with AOD issues, through assessing need, identifying treatment options and creating holistic treatment pathways.
  • Part of this stream also includes providing interim support to clients who are awaiting assessment and treatment commencement to assist them to remain engaged and keep on track.

 

The model of service delivery is underpinned by the Community Integration Model (CIM), an approach to practice which increases ‘recovery capital’ for individuals.
