Out of home care
Welcome to SalvoCare Eastern Home Based Care Program, based at Mt Martha.
Here at Mt Martha we run two Programs. A Home Based Care program, providing Foster Care through DHS referred, and an Adolescence Support Program providing assistance to teens who are struggling with issues in the home, and have been refered by DHS as an intervention to prevent the young person from being removed from the family home and placed in Home Based Care (Foster Care).
All of our staff work with both HBC(Home Based Care) and ASP (Adolescent Support Program) clients. Each of our case Managers work with both the carer and the young person to support the placement within that family.
